If you decide that you would like to begin therapy, I’ll ask you to complete the following forms for our first appointment:
- Intake Questionnaire
- Information For Clients
- HIPAA Authorization
- Minor Confidentiality (only if you are under 18 years old)
- Release of Information (for each person you want me to be able to talk to)
Fees and Payment
I do not bill insurance directly, which results in me having significantly more time to focus on you, to coordinate with your other health care professionals, and to provide you with the most comprehensive and effective treatment possible. Payment is accepted at time of service through cash, check, or credit card. My customary rate is $100 for a 50-min session. You will receive a monthly statement including all information necessary for you to submit a claim to your insurance company for out-of-network benefit reimbursement directly back to you. While I cannot guarantee coverage or reimbursement, I would be happy to provide any information necessary to assist you in understanding your benefits and submitting claims.
